From 31 May 2023, General Practice can send referrals to Canterbury Hospital Maternity and the Concord Hospital Midwifery Group Practice using HealthLink SmartForms from Best Practice, Genie, Medical Director. Practices without a compatible practice software system can use the MyHealthLink Portal or fax a referral using this coversheet.
Using HealthLink eReferrals enables:
The eReferral form replaces the need for patient booking requests, but women can register their interest to CAPS and MGP models of care on the numbers below. Registering with CAPS and MGP are not replacements for a GP referral to the service.
