The CESPHN care finder network consists of 10 organisations working throughout our region to deliver specialist and intensive assistance to older Australians who need extra intensive support to access aged care services and other supports.
The care finder service targets the most vulnerable older people in our community who could otherwise fall through the cracks. The care finder service is a free service which supports vulnerable people who have no one else who can support them, to learn about, apply for and set up aged care and other support services.
