GPCanShare specialist nursing and allied health teams are now well established across the Central and Eastern Sydney PHN region. These teams facilitate seamless coordination of care for patients and liaise between the patient’s cancer specialist team and their GP by:
Due to the success of the local GPCanShare teams, the region-wide GP Cancer Phone line will be discontinued from 31 December 2022.
GPs can continue to access cancer information (service navigation and referral pathway-treatments, symptom management -palliative care) by contacting a GPCanShare Clinical Nurse Coordinator or Social Worker Monday – Friday 8:00am – 4:30pm at the below hospitals:
