The Comprehensive Health Assessment Program (CHAP), designed for conducting Annual Health Assessments for people with intellectual disability, is now free for anyone to use. The Department of Health recently acquired the rights to the tool from University of Queensland and is actively working on integrating it into GP software. In the meantime, the CHAP is available as an editable .pdf for anyone to use.
Project GROW program offers a full program of initiatives that support and encourage the uptake of annual health assessments and promote preventive healthcare for patients with intellectual disability.
