Calling General Practices to Participate in the Activating Primary Care COPD (Chronic obstructive pulmonary disease) Patients with Multimorbidity (APCOM) trial.
The trial aims to implement and evaluate an innovative tailored, self-management support program, delivered by Practice Nurses to people with COPD and comorbidities and has received ethics approval.
