Hepatitis B Pathways to Care is here to help GP’s and their patients in Sydney Local Health District (SLHD).
The program provides all GPs with patients living in SLHD an opportunity to optimise care and support in hepatitis B, to avoid longer term the profound consequences of liver cancer, cirrhosis, and end-stage liver disease.
Many GPs and their patients have already participated in the Hepatitis B Pathways to Care program. The program will be ongoing in 2023.
Through the program, SLHD Gastroenterology and Liver (G&L) Service supports GPs where needed in:
Development of a management plan
Assessment and referral for antiviral treatment, if required
Cirrhosis care
FibroScans
The G&L service will also support patients directly with education on living with Hepatitis B, reinforcing the importance of monitoring and treatment, and reducing transmission risk.
The Public Health Unit can also counsel family members and other contacts regarding Hepatitis B risk and encourage them to attend primary care providers for screening, vaccination, or care as appropriate.
You can find detailed information about the program in this flyer. For further information please email SLHD-CDteam@health.nsw.gov.au or call the PHU on 95159420
