Central and Eastern Sydney Primary Health Network (CESPHN) has launched two programs for older adults living in the community and in residential aged care facilities (RACFs).
Program for older adults affected by COVID-19
Relationships Australia NSW has been funded to provide the Reconnect Wellbeing Program, which aims to reduce social isolation and loneliness, improve wellbeing and increase connections for older people affected by COVID-19.
This service is specifically designed for people over 65 years and Aboriginal and Torres Strait Islanders over 55 years who live in the CESPHN region. It offers client-centred casework support, low intensity psychological therapies, psychosocial support, and referrals to other health and support services.
GPs, allied health workers and pharmacists are uniquely placed to understand and be attuned to the changing needs of their patients/clients with whom they provide health advice and medications. Those affected may present as experiencing loss, isolation, and disconnection.
Anglicare has been funded to provide mental health services to older people who reside in Residential Aged Care facilities. Emotional wellbeing for older adults program aims to provide psychological and psychosocial therapies.
Residents experiencing mild to moderate mental health issues as diagnosed by a GP, a psychiatrist, a RN or allied health staff are eligible for this program. This also includes residents who may be ‘at risk’ of developing a mental illness if they do not receive appropriate treatment.
Referrals can be made by facility care managers/DONs/RNs who can also support GPs visiting facilities to refer.
