My Health Matters Evaluation Survey

The Council for Intellectual Disability (CID) created the My Health Matters folder in 2018, in response to a UNSW study into preventable deaths.  Since then, 2000 folders have been distributed amongst the intellectual disability community across the world.

The folder was create with the intention to mutually improve communication between health and people with intellectual disability and in doing so, have a positive effect on health outcomes.

CID are currently conducting research around the effectiveness of the resource and looking to talk with health professionals.

If you’ve had a patient use My Health Matters during an appointment we would like to hear from you.

There are 3 ways of participating:

  1. fill in our quick survey here via Survey Monkey
  2. participate in a 1 on 1 interview (over phone or Zoom) or
  3. Share this email within your health networks.

If you participate in a 1 on 1 interview we will remunerate you for your time.
