CESPHN – Support for nurses working in general practice

Are you a new nurse working in general practice?

Our practice support team are available to provide you with a Practice Nurse orientation session which includes a copy of our Practice Nurse Resource Guide.

We can also provide you with ongoing support to help you learn and succeed in your new role.

These sessions can either be an in general practice or a virtual visit.

HealthPathways for Practice Nurses

HealthPathways is a free web-based information portal to support primary care clinicians to plan and manage patient care.

The platform contains concise and locally-adapted pathways, for use during a consultation, to help navigate community and hospital services and provide the best health outcome for patients.

This resource is available to locally registered healthcare providers.

Central and Eastern Sydney PHN covers two HealthPathways regions:

  1.  South Eastern Sydney – which covers referral options for South Eastern Sydney LHD services including Sutherland, St George, Prince of Wales/Royal Hospital for Women, St Vincents Hospital and Sydney Children’s Hospital network
  2.  Sydney – which covers referral options for Sydney LHD services including RPA, Concord, Canterbury, and Balmain hospitals.

Expression of Interest (EOI)

Representation on the Domestic, Family and Sexual Violence (DFSV) Program Advisory Committee

CESPHN is seeking expressions of interest from practice nurses who would like to join our Domestic, Family and Sexual Violence (DFSV) Program Advisory Committee.

Since 2020, CESPHN has been funded by the Department of Health and Aged Care to improve primary care responses to domestic and family violence (DFV) through training, referral support and system integration and improvements.

In 2023, CESPHN received further funding to expand the program to include Sexual Violence and Child Sexual Abuse.

CESPHN – Vax at Home Program

“You don’t have to wait 6 months anymore between your last infection and having a COVID-19 vaccination”.

The CESPHN Vax at Home Program provides:

  • At home COVID-19 vaccination for people who are housebound and not able to leave their house to access a COVID-19 vaccination or booster, aged 16 years and over.
  • In-reach COVID-19 vaccination clinics at Residential Aged Care Facilities (RACFs).

How to access the service:

  • Housebound patients: Upload a GP or Local Health District referral to the Vax at Home Service form. Patient self-referrals will not be accepted. For enquiries on this service, please contact vaxathomeservice@cesphn.com.au
  • Residential Aged Care Facilities: Book a vaccination clinic by emailing racfs@cesphn.com.au

The Vax at Home service aims to ensure that all housebound patients and RACF residents can receive their COVID-19 booster dose in accordance with the 2024 ATAGI recommendations.

New Practice Guide – Managing uncertainty in professional practice

The Australian Institute of Family Studies have released a new guide which outlines what uncertainty and uncertainty tolerance are and their significance to professional practice.

It also outlines the individual, practice and organisational factors that can contribute to uncertainty and provides evidence-based strategies for uncertainties.
