Every quarter the Digital Health Team at CESPHN provides feedback to practices through a Practice Progress Report for those partaking in the Quality Improvement PIP QI program, reflecting on their data quality improvements over the previous three months.
Most practices improve one Quality Improvement Measure (QIM) with some improving a few quite dramatically. We often we see data where the team has clearly worked exceptionally hard to maintain or improve from a low base to high quality.
CESPHN would like to acknowledge these practice teams for their efforts by presenting them with a “Certificate of Commendation”.
This quarter, we presented a Certificate of Commendation to Dr Haque and his team at Medical Centre Southpoint for consistently high data quality especially accreditation data, where they regularly score 100%.
We asked Dr Haque to share his thoughts on how he achieves great data.
“Quality data involves a contribution from everyone in the practice. At reception, the patient is encouraged to share as much information as possible in the patient registration form without pressure. Nurses are trained to identify gaps and try to complete this in a private clinical setting before the patient sees the GP in the consulting room. Lastly, GPs can explain why certain data is needed and explain the benefits to the patient. As you can see, there are three opportunities at each visit to canvas data”.
Dr Haque has also agreed to act as Clinical Lead in the second iteration of the Person Centred Medical Neighbourhood Program (PCMN). We look forward to his valuable input on clinical and data issues for the program.
