Royal Prince Alfred Hospital’s new Emergency Hospital in the Home (EHITH) Service has been active since November 2022. The service provides rapid assessment and management of acute medical conditions to older community dwelling patients in the safety of their own home, allowing patients to avoid unnecessary visits to the emergency department. General Practitioners are a vital source of referrals to our service. Here we discuss an example of a referral.
G is a 66 year old gentleman with end stage multiple sclerosis. He is bed-bound and supported by his wife and carers at home. He has a long-term suprapubic catheter for neurogenic bladder, complicated by recurrent urinary tract infections which have previously required hospital admissions for catheter changes and intravenous antibiotics.
When G appeared to be developing signs and symptoms of another urinary tract infection, he was referred to the EHITH service for review. Given the terminal stage of his disease, G’s family were keen to avoid hospitalisation as it can often be quite distressing and often exacerbates delirium. EHITH were able to perform a home visit on the same day, change the suprapubic catheter, collect pathology and commence empirical intravenous antibiotics whilst allowing G to remain in familiar surroundings with his family.
