St Vincent’s Hospital Sydney has a new diabetes in youth clinic!

Our Diabetes in Youth (DIY) clinic is for young adults living with diabetes, aged between 16-25 years old, who are transitioning from paediatric to tertiary adult diabetes service.

This transition time to adult services can be difficult. Unsuccessful transitions can result in presentations to ED with avoidable DKA or Hypoglycaemia.

Successful transition between paediatric and adult healthcare services requires early, individualised planning. This is best achieved with a model of care that empowers young people to manage their diabetes amid the challenges of young adult life.

The DIY multidisciplinary team includes an Endocrinologists, Diabetes Educator, Dietitian and Exercise Physiologist.

The clinics are run every three weeks on Tuesdays afternoon between 2.30pm-6.00pm. Telehealth & Face-to-face consults are available.
