The NEW PHN online mental health and psychosocial support referral forms will go live on Monday 12 July.
This includes referrals to the following programs:
CESPHN, working with our client management system vendor (rediCASE), will release the new referral forms over the weekend of Saturday 10 and Sunday 11 July and the new links will be available on Monday 12 July from 9.00 am.
Referral form not available over the weekend
The referral forms will not be available while the system is updating, and which will be from 6pm Friday 9 July until 9.00 am on Monday 12 July.
As these improvements have been designed to simplify the referral process, changes to the look are minimal.
New Referral Form Link for Connect and Thrive
On Monday12 July there will be a new referral form link for Connect and Thrive and we will update our website with this new link.
The link for PSS and PICS will remain the same.
