The Vax at Home Program was introduced in 2021 during the COVID-19 global pandemic. Throughout the existence of the program, CESPHN along with its providers have supported the most vulnerable people in our community.
The program was initially developed to support housebound people who could not leave their home to access a COVID-19 vaccination. Over the last three years, the need to additionally support residents in residential aged care home (RACH) was identified. Our team liaised with GPs, their patients, aged care home staff and providers to ensure as many vulnerable people could access the COVID-19 vaccine.
The Vax at Home program has delivered nearly 700 COVID-19 vaccinations to housebound patients and close to 9,000 vaccinations have been delivered to residents in residential aged care homes.
We are very proud of the work achieved through this service and the number of people supported. This program has been one of the most successful and rewarding programs we have implemented due to the COVID-19 pandemic.
Program conclusion
The Vax at Home Program will be ceasing at the end of September 2024. The last day for GP referrals for housebound patients that need a COVID-19 Vaccine or booster, will be Wednesday, 18 September 2024.

For Residential Aged Care Homes – final requests for COVID-19 clinics are to be emailed to vaxathomeservice@cesphn.com.au by 18 September 2024.
We would like to reiterate that the risk of serious illness from COVID-19 increases with age. The rate is higher for those with other serious health conditions or a weakened immune system. Please continue to encourage your vulnerable patients to access a COVID-19 vaccine or booster at the appropriate time for them.

Patients will be able to access the COVID-19 vaccine and boosters through their local GP or Pharmacist. To find a local COVID-19 vaccine clinic, please support your patients to go to Healthdirect and ‘Find a health service’ if your practice does not participate in the program.
