Objective: To focus on high priority groups to improve early and enhance access to services relating to lifestyle modification to meet the needs of our local communities. This will be achieved through collaborations, pathways and partnerships with key stakeholders, commissioned initiatives and workforce education and training.


Adopting a healthy lifestyle is an important element of chronic disease management.

Living a healthy lifestyle includes:

  • Eating nutritious foods
  • Regular physical activities
  • Avoiding excessive alcohol consumption
  • Avoiding tobacco use
  • Achieving and maintaining a healthy body weight and a healthy waist
