Population Level Analysis and Reporting (POLAR) is a cloud-based data extraction tool for general practice. It is provided by Outcome Health. POLAR securely extracts data from practice software. This data is used by practices and CESPHN to support quality improvement and business development, providing better patient-centred care.
Use of a data extraction tool allows your practice to easily de-identify your electronic medical records and then share it with CESPHN.
If you intend to participate Practice Incentives Program Quality Improvement Incentive (PIP QI) you must have a data extraction tool installed. Contact digitalhealth@cesphn.com.au for support to register your practice.
