Faxed, mailed, and emailed referrals will no longer be accepted.

Women with T1DM or T2DM will require an eReferral as soon as the pregnancy is confirmed and will need to be seen at 8 weeks gestation with the Diabetes in Pregnancy team.
All referrals are triaged by a midwife to ensure women receive care at the appropriate time by the appropriate clinician. To facilitate the triage process please provide a detailed medical, surgical and obstetric history (including any history of LLETZ, cone biopsy, preterm birth or miscarriage) in the referral.
Ensure that all results that accompany the eReferral have the womens identifying details on EVERY PAGE.

Fetal movements
Enquire and educate women about fetal movement at every antenatal visit from 20 weeks gestation. If the clinician determines any concerns with FM contact the birth unit triage midwife on 0439 869 035 to arrange for an urgent review. For further information refer to the following resources:
Women with abnormal OGTTs should be immediately referred to the Diabetes in pregnancy clinic. Women should be seen in the clinic within a week of diagnosis of GDM. Please email the pathology results and referral to the diabetes educator on SESLHD-RoyalHospitalforWomen-DiabetesServices@health.nsw.gov.au

Updated documentation on the yellow card is the key to successful shared care. When investigations are performed by the GP ensure all results are documented on the front of the yellow card. Document details of every antenatal visit on the back of the card.
The yellow card becomes an important official hospital record, and it should be kept as complete as possible.
