EOI: GP Pharmacist Advisor for GP+ Program

Central and Eastern Sydney PHN (CESPHN) is seeking a skilled and experienced GP Pharmacist to provide advice on the implementation of the GP+ Program.

The GP+ Program aims to embed multidisciplinary teams into general practice to support health needs of patients with chronic diseases, especially those from priority population groups.

CESPHN’s has commissioned a non-dispensing consultant pharmacist to work across several smaller and solo general practices within the St George region. The pharmacist will deliver in-practice services including the support of chronic diseases and complex health conditions, through medication management, reviews and quality improvement activities.

This advisory role is separate to the commissioned pharmacist and will focus on providing advice to CESPHN on the implementation of the GP+ Program.

Key Responsibilities for GP Pharmacist Advisor

  • Provide expert advice and guidance to CESPHN’s project team to support the implementation and ongoing development of the GP+ Program
  • Contribute to the development of resources, training materials and onboarding processes to support the integration of pharmacists into general practice settings
  • Provide insights on general practice workflows, medical software and booking systems relevant to the pharmacist role
  • Advise on strategies to support pharmacist engagement and role clarity within general practice.
  • Assist in the development of quality improvement activities for the GP+ Program.
  • Provide feedback and recommendations to inform continuous improvement of the GP+ Program.

Selection Criteria

  • At least 2 years’ experience working as a general practice pharmacist
  • Relevant pharmacist qualification with current AHPRA registration
  • Strong understanding of medical software, booking systems and general practice workflows
  • Experience in development and implementation of quality improvement activities
  • Excellent communication skills with capacity to collaborate with GPs and allied health clinicians
  • Experience providing strategic advice or contributing to program or service implementation within a primary care or general practice setting

Time Commitment

  • Time required may vary depending on project stage, with an average of 4 hours per month

Remuneration

The GP Pharmacist Advisor will be remunerated for their time and appointed in accordance with the terms and conditions outlined in the EIS representation policy.
