Intellectual Disability Primary Care Inclusion Program 

This Expression of Interest (EOI) is for general practices in the Central and Eastern Sydney PHN (CESPHN) region to participate in the Intellectual Disability Primary Care Inclusion Program. This program aims to support and build capacity of primary care providers to effectively care for patients with intellectual disability and assist their families and carers. 

The Intellectual Disability Primary Care Inclusion Program is an activity of CESPHN’s Project GROW. Project GROW is a component of the National Roadmap for Improving the Health of People with Intellectual Disability and focuses on improving health care for people with intellectual disability, their families and carers.  

The Intellectual Disability Primary Care Inclusion Program provides $5000 in financial support to general practices for participation in Quality Improvement activities and in-practice training related to intellectual disability health care. Funding will be distributed in two payments of $2,500 (excluding GST). The first payment will be made upon execution of the Memorandum of Agreement (MOA) and commencement of the program. The second payment will be issued upon program completion. 

How to apply  

Practices must submit an Expression of Interest (EOI) to apply for the program. Applications are to be submitted via the online form.

Practices who have not participated in this program before will be given priority. If funding permits those practices who participated previously will be considered.

Classification Questions from 2021-2023

Q. We are interested in joining the program for multiple locations within the CESPHN. We would like to know- if there is a minimum number of practice staff, GP or nurses that is required for those training sessions and QI activities?

A. No, there is no minimum participation requirement. The program is open to the entire practice, and we encourage all relevant staff, including GPs, nurses, administration staff and allied health clinicians to participate.

If multiple practices are applying, each practice must submit a separate EOI to be eligible for the $5,000 funding per site. If you prefer to combine the program across locations, only one EOI is required, but funding will be limited to a single payment of $5,000.

Q. Does the practice require the presence of a practice nurse to undertake the Intellectual Disability Primary Care Inclusion Program.

A. No, a practice nurse is not required. The practice only needs the capacity to undertake training and complete Quality Improvement activities.
