Job Title: Practice Nurse
Skills Experience:

Key Responsibilities
Deliver comprehensive nursing care within the general practice setting, including wound care, chronic disease monitoring, immunisations, and routine clinical assessments.
Undertake patient education and health promotion, supporting self-management plans for conditions such as diabetes, COPD, asthma and cardiovascular disease.
Perform screening, triage, and point-of-care testing, ensuring accurate documentation and follow-up.
Collaborate with GPs and allied health professionals to coordinate patient care and improve clinical outcomes.
Maintain infection control standards, clinical governance and contribute to quality improvement activities.
Support administrative tasks related to nursing workflows, recalls, and immunisation registers.
What We’re Looking For
Qualifications: Bachelor’s degree in Nursing and current AHPRA registration as a Registered Nurse.
Experience: 3–6 years post-registration experience, with demonstrated practice nursing experience in general practice or primary care.
Registration & Eligibility: General registration with the Australian Health Practitioner Regulation Agency; Australian citizen or eligible to work in Australia.
Strong clinical assessment skills, excellent communication, and an ability to work independently and within a multidisciplinary team.
Commitment to patient-centred care and continuous professional development.
