Residential Aged care home Support

Central and Eastern Sydney PHN provides funding and support to various Geriatric Flying Squads and Outreach Services operated by partner Local Health Districts and Hospital Networks. These services are intended to reduce the need for transfer to hospital by managing residents at their home.

Aged Care On-site Pharmacist

CESPHN is supporting the new Aged Care on-site pharmacist initiative in residential aged care. The initiative care started on July 1 2024, with the aim of improving medication management and safety in residential aged care homes.

The initiative aims to:

• Improve medication use and safety, including the safe use of high-risk medications.
• Provide continuity in medication management with day-to-day reviews and prompt issue resolution.
• Offer easy access to pharmacist advice for residents and staff and
• Integrate pharmacists with the healthcare team, including local GPs, nurses, and community pharmacies.
