The annual CESPHN Primary Health Awards aims to recognise primary health professionals and organisations that have provided significant and exceptional service or an outstanding contribution to primary health care during the 2021-22 financial year.
Improving integration of care: This award is for individuals/organisations who have contributed to building stronger local partnerships, better communication between local providers and a better experience of care.
Improving practice in primary health care: This award is for individuals/organisations in primary health care that demonstrate improvements in service quality and patient experience.
Service delivery that meets local needs: This award is for individuals/organisations whose work has addressed local gaps and needs to improve health equity through high quality service delivery.
Each category can cover achievements across a range of endeavours including the delivery of primary healthcare, commissioning, teaching and training, research or community work.
Award guidelines
The criteria for each of the award categories is:
The activity must have been undertaken in the 2021-22 financial year.
The activity must have focused on a community or communities within the CESPHN region
The individual/s involved must be of good character.
In regard to the activity, the following must be outlined in the application:
What was the problem/health need (that aligns with the relevant category) that the nominee tried to solve?
How did the nominee approach it?
What were the outcomes for the community or the practice?
Supporting evidence should be provided. This can be:
