Palliative Care General Practice Education and Quality Improvement Program
This Expression of Interest (EOI) is for general practices in the Central and Eastern Sydney PHN (CESPHN) region to participate in the Palliative Care General Practice Education and Quality Improvement (QI) Program. This program aims to support the general practice to build knowledge and capacity to provide more effective care for palliative patients, their families and carers.
The Palliative Care General Practice Education and QI Program provides $5000 in financial support to general practices for participation in in-practice education and QI activities related to Palliative care. Funding will be distributed in two payments of $2,500 (excluding GST). The first payment will be made upon execution of the Memorandum of Agreement (MOA) and commencement of the program. The second payment will be issued upon program completion.
How to apply
Practices must submit an Expression of Interest (EOI) to apply for the program. Applications are to be submitted via the online form
