Primary health Awards

The annual Central and Eastern Sydney Primary Health Network (CESPHN) Primary Health Awards recognise primary health care professionals and organisations that have provided exceptional service or an outstanding contribution to primary health care in the Central and Eastern region of Sydney during the previous financial year.

2025 Primary Health Awards

The fourth annual Primary Health Awards will be held at Aqua Luna in Drummoyne, Sydney in October 2025. Further information on nominations will be available soon.

Award categories

Award categories are for:

Demonstrate improvements in service quality and patient care.

Demonstrate improvements in service quality and patient care.

Open to community service organisations. Demonstrate improvements in service quality and care focused on meeting community needs.

Open to Aboriginal and Torres Strait Islander organisations, community based, private or a public sector organisation that have made a positive contribution to developing the health and wellbeing of their community in which they serve. Can include Aboriginal or non-Aboriginal persons.

Award guidelines

All of the awards include individual or group nominations. The service must be located in the CESPHN region and activity undertaken during the previous financial year.

The following criteria applies to the awards for:

Excellence in General Practice

Excellence in Allied Health Practice

Excellence in Community Service Delivery

A brief description of the service/program/person nominated for the award (max 100 words)
What was the health need or issue that the nominee addressed? (max 100 words)
Explain the intervention (max 300 words)
What were the measurable, demonstrated service quality improvements or patient/community outcomes? (max 300 words)
Support material including images can be provided if available, but this is not necessary (max 1 page)

The following criteria applies to the Excellence in Aboriginal Health and Wellbeing Achievement:

How did the nominee demonstrate respect, build sustained and inclusive relationships and/or create opportunities for Aboriginal and/or Torres Strait Islander communities or individuals?
How did the nominee’s activity make a difference to reconciliation and healing in a culturally competent and safe manner?
What is the nominee’s connection to the CESPHN region?

Please note that EIS Health Board Members and members of CESPHN’s Clinical and Community Council are ineligible to apply.

Three nominations are shortlisted in each category by an independent judging panel. All shortlisted nominees are invited to attend the awards ceremony.
