GP Activation Series for Chronic Conditions Management

Join the CCM Activation Series! Central Eastern Sydney PHN invites your practice to prepare for the biggest changes in Chronic Conditions Management (CCM) in 20 years, effective from 1 July 2025.

Activity 1: If you missed activity 1, access it here. 

Activity 2: Team Awareness & Role Exploration

  • Goal: Raise awareness of MyMedicare and CCM changes.
  • Action: Define team roles in MyMedicare and CCM.

Activity 2 Details:

  • Objective: Understand MyMedicare and CCM changes.
  • Outcome: Clear team roles and responsibilities.
  • Ideas:
    • Team meetings or lunch catch-ups.
    • Staff room updates.
    • Informative emails.

Benefits of MyMedicare:

  • Strengthens patient-practice relationships.
  • Supports integrated, person-centred care.

CCM Changes:

  • New MBS items from 1 July 2025.
  • Focus on regular reviews and ongoing care.
  • Enhanced roles for practice nurses and health practitioners.

Get Involved:

  • Register for MyMedicare.
  • Communicate with and register patients.
  • Review and adapt practice processes.
