Request for Proposal: Provide consultant pharmacist services for the GP+ Program
CESPHN invites suitably qualified providers to deliver general practice (GP) pharmacist services to smaller and solo general practices for the GP+ Program through a ‘Request for Proposal’ (RFP) process.
A GP Pharmacist in this instance refers to a non-dispensing, consultant pharmacist who works in general practice, in collaboration with GPs.
GP+ is an innovative program funded by the Department of Health and Aged Care, aiming to integrate multidisciplinary teams into general practice. The GP+ Program aims to encourage greater use of team-based are, increase the use of allied health providers and reduce avoidable hospitalisations. GP+ will target patients with chronic diseases, particularly those from vulnerable population groups.
The GP+ program requires the provision of GP pharmacist services up to the value of 1.0 FTE, which will initially service 5-10 general practices. The GP Pharmacist will be a shared resource across participating general practices in the St George region and will be co-located onsite at the practice.
The GP Pharmacist will deliver non-dispensing pharmacist services including medication reviews and quality improvement activities, targeting patients with chronic diseases, particularly those from vulnerable population groups.
The scope of service will vary depending on requirements of the general practice, local needs and demographics.
