Requesting proposals for social work services for the GP+ Program
The GP+ Program aims to embed multidisciplinary teams into general practice to support health needs of patients with chronic diseases, especially those from priority population groups. The program is targeted to solo and smaller general practices to encourage greater use of team-based care, increase the use of allied health providers and reduce avoidable hospitalisations. The program will target chronic diseases and health conditions that are amenable to a multidisciplinary approach and support vulnerable populations unable to afford private allied health fee paying or co-payment services.
The GP+ program will involve the commissioning of a social worker, which will be a shared resource across participating general practices in the St George region. They will be co-located onsite within general practice, to maximise opportunities to build relationships and establish trust between allied health clinicians, general practitioners and patients.
The social worker will deliver in-practice social work services within participating general practices including the provision of psychosocial support and assistance with system navigation of health and social systems. Scope of practice may vary slightly between general practices, dependent on local patient needs and demographics, and will be determined through collaboration with GPs prior to commencement of service.
