St George Hospital Clinical Nurse Consultants (CNCs)
If you have a patient that has had a hospital presentation and has now been returned to the care of your general practice, the amazing CNCs are available to help answer any questions you may have about your patients and their care moving forward.
CESPHN funds a range of mental health and psychosocial services to support individuals. All services are person-centred and targeted to support the person’s individual needs, applying a stepped-care approach to find the program that is most suitable for the level of support required. Services are appropriate for a range of community and cultural groups and are offered both online and in person throughout the region.
Approximately 224,000 people, or 14 per cent of people in our region are aged 65 years and over. This number is predicted to increase a further 43% to 320,000 by 2031. To support the health needs of our older people, Central and Eastern Sydney PHN are providing a number of programs and initiatives focused on improving access to high quality primary healthcare.
We hope you are excited to celebrate primary health care nurses on Wednesday, 5 February.
The first Wednesday in February each year is Primary Health Care Nurses Day, where we celebrate the amazing work that nurses do outside of the hospital setting, in aged care, community, general practice, custodial, schools and a range of other primary health care settings.
This year’s campaign is ‘Treating the whole person’.
CESPHN would love to see how you celebrate Primary Health Care Nurses Day so send us a picture for us to share on our social media pages – practicesupport@cesphn.com.au
