Note: This program is currently only available to solo and smaller (2-4 GPs) general practices in the St George, Bayside, Randwick and Sutherland LGAs.
This Expression of Interest (EOI) is for general practices in the St George, Bayside, Randwick and Sutherland regions to participate in the GP+ Program.
GP+ is an exciting, new program which offers an innovative approach to multidisciplinary team-based care in the primary care setting. The GP+ Program offers practices the opportunity to have a non-dispensing consultant pharmacist and a social worker to work in their practices on a part-time basis (approximately two sessions per week). These allied health services will target patients with chronic conditions, with priority to those from vulnerable population groups.
Practices will benefit from access to psychosocial support and service navigation for patients with complex needs (e.g. My Aged Care, housing assistance, Centrelink etc.), as well as individualised consultant pharmacist support for patients to manage their chronic conditions. General practices are encouraged to host both the pharmacist and social worker at their practice, however preference for one service can be accommodated.
These services will be available for eligible patients at no cost.
This program is ideal for small practices that value:
Holistic patient care and continuity of support
Collaborative, team-based approaches to managing chronic disease
Improving the patient experience within their practice
Strengthening access to allied health
The GP+ Program is funded by the Department of Health, Disability and Ageing. Financial support to the value of $5000 (excluding GST) will be offered to general practices to participate in the program and will be required to sign a Memorandum of Agreement (MOA).
