MyMedicare is a voluntary patient registration model introduced by the Department of Health and Aged Care. It formalises the relationship between patients, their general practice, GP, and primary care teams. Evidence shows that seeing the same GP and healthcare team regularly leads to better health outcomes.
Benefits of MyMedicare
For Patients:
Continuity of Care: Ensures consistent and coordinated care with a formal relationship with their GP.
Access to Services: Enhanced access to telehealth services for children under 16, pensioners, and concession card holders.
For Practices:
Incentives: Access to incentives such as bulk billing for longer telehealth consultations for children under 16, pensioners, and concession card holder.
Improved Patient Management: Better tools and resources for managing patient care and improving health outcomes.
General Practice in Aged Care Incentive (GPACI)
The GPACI aims to enhance the quality and continuity of care for residents in residential aged care homes.
Benefits of GPACI
For Patients:
Regular Visits: Ensures residents in residential aged care homes receive regular visits and care planning services from their responsible GP and practice.
Continuity of Care: Supports continuous, quality care and aims to reduce avoidable hospitalisations.
For Practices:
Incentive Payments: Quarterly incentive payments for services to registered patients in residential aged care homes.
Financial Support: Payments of $300 per patient per year for GPs and $130 per patient per year for practices.
These initiatives support healthcare providers in delivering high-quality, tailored care to keep patients with complex, chronic diseases out of hospitals.
