Central and Eastern Sydney Primary Healthcare Network (CESPHN) is seeking expressions of interest (EOI) from Residential Aged Care Homes (RACHs) interested in participating in an Aged Care Onsite Pharmacist (ACOP) Transition Grants Program
The program provides structured funding to support RACHs to embed onsite pharmacists with multidisciplinary care teams. It aims to strengthen medication safety, enhance digital capability, and build workforce readiness across both multisite providers and smaller single site organisations.
RACHs participating in the Program will be eligible for up to $6,250 to undertake activities that prepare them for successful implementation of ACOP services, under Tier 1 or Tier 2 arrangements.
Eligible organisations may apply for the grant under one of two applicant types:
Single‑site RACH organisations – those operating one RACH within the CESPHN region, may submit an application for that single facility.
Multi‑site aged care organisations – operating two or more RACHs within the region, may choose to apply by submitting separate applications for each facility, with each site receiving individual funding and separate activities.
Participating RACHs will have 12 months from the date of signing a grant agreement to complete funded activities.
This grant opportunity is limited to up to 50 RACHs within the CESPHN region.
