Concord Repatriation General Hospital: Updates to how GPs receive hospital documentation

Concord Hospital will no longer be sending Ambulatory Care patient letters by post. GPs can access hospital documentation via the patient’s MyHealthRecord or contact Health Information Record Service (HIRS): Ph. 9767 5451, Fax. 9767 6651 or Email:SLHD-ConcordOngoingCareRequests@health.nsw.gov.au

To ensure your practice receive electronic documentation in the future, confirm you are the GP listed against that patient (request your patient checks their record at each visit).

If you are correctly listed as the patient’s GP and still not receiving hospital communication delivered electronically to your practice software:

  1. We recommend the use of Medical Objects. There is no cost for a receipt only subscription and this can be done alongside existing secure messaging accounts.
  2. Alternatively, if your practice is registered with HealthLink, contact the HealthLink Support Desk to escalate the issue noting NSW Health information is not being transmitted through to the practice.
