As the National Mental Health eReferral SmartForm is a single referral form used for a range of mental health services, it is important to complete all sections and clearly identify the service you are referring to. If you are unsure of the specific service, please indicate the type of support required. Providing complete information helps ensure your patient is directed to the most appropriate service.

Incomplete referrals will result in delays, requests for further information, or referral closure.
Providing complete referral information from the outset helps us triage accurately, connect patients with the right service sooner, and improve their access to care.
