Excellence in primary health care celebrated at CESPHN’s Primary Health Awards

Central and Eastern Sydney Primary Health Network (CESPHN) is proud to announce the winners of the 2025 Primary Health Awards, recognising excellence, innovation and quality improvement in primary health care across the region.

The annual awards recognise primary health care professionals and organisations’ that provide an exceptional service, an outstanding contribution or a quality improvement to primary health care in the central and eastern region of Sydney. This year, the awards night also celebrated CESPHN’s 10 year anniversary.

The awards ceremony took place on Thursday, 23 October 2025, celebrating individuals and organisations making a profound impact in their communities.

This year’s winners stood out among a diverse field of finalists across four categories:

Excellence in General Practice: ADHD @ Your GP – Kirrawee Family Medical Practice
This groundbreaking initiative has transformed ADHD care in the Sutherland Shire. By empowering GPs with enhanced training and a team-based approach, ADHD @ Your GP offers neurodivergent-friendly assessment, diagnosis, and management for both adults and children. The program has expanded prescribing capacity, reduced wait times, and improved continuity of care for over 320 patients.

Excellence in Allied Health Practice: MediLearn
MediLearn delivers specialised medication education for aged care workers, addressing a critical gap in aged care training. Led by clinical pharmacists, the program offers flexible, accessible learning tailored to the unique challenges of medication management in aged care settings. MediLearn’s innovative model supports safer practices and empowers staff to deliver better care.

Excellence in Community Service Delivery: Waterloo Peer Education Program, Sydney Local Health District and Counterpoint Community Services 
The Peer Educator Program at Waterloo estate – one of Australia’s largest social housing communities – was a community-led initiative aimed at addressing persistent health inequities. Residents were trained as peer educators in topics like mental health, domestic violence, nutrition, and drug issues. They then redesigned and delivered this information through engaging workshops to over 800 fellow residents across two years. Evaluated by the Health Equity Research and Development Unit at Sydney Local Health District, the program was found to significantly improve social connections, mental health, and overall wellbeing among participants.

Excellence in Aboriginal Health and Wellbeing Achievement: Tribal Warrior Aboriginal Corporation
Tribal Warrior’s Women’s Health & Wellbeing Program offers culturally safe fitness and empowerment activities for Aboriginal women. With a holistic, trauma-informed approach and strong community partnerships, the organisation promotes healing, resilience, and connection. Their work continues to strengthen emotional wellbeing and build pathways to better health.

Each winner receives $2000 to further their work, regional promotion and a luxurious overnight stay for two at any Ovolo Hotel in Australia.
