Join us for an Allied Health-specific webinar exploring the upcoming changes to the Chronic Conditions Management framework under MyMedicare.

From 1 July 2025, GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) have been replaced by a streamlined GP Chronic Condition Management Plan (GPCCMP). These updates simplify referral processes and remove the need for multidisciplinary collaboration agreements, making it easier for allied health professionals to support patients with chronic conditions.

This session is essential for allied health professionals delivering MBS services under chronic disease management.
