The Central and Eastern Sydney PHN (CESPHN) is inviting community members, and health and aged care providers to attend one of our ‘Care Finder’ Industry Briefing webinars scheduled for Wed 27th July and Wed 3rd August 2022.
What is the Care Finder program?
The Care Finder program is a Commonwealth funded aged care initiative. The program is part of the Governments response to the Royal Commission into Aged Care Quality and Safety’s recommendation for more localised and face-to-face supports to help connect older people to aged care services. CESPHN will be providing support to Care Finders in our region through the funding to Care Finder organisations. These organisations will help older people requiring more intensive assistance navigate the My Aged Care system and access aged care services and other supports. Service delivery is expected to start from Jan 2023.
Purpose of the briefing
The Industry Briefing will involve a short presentation about what the Care Finder program is, the changes to specifically target older people requiring more intensive specialist support, and how such services will be commissioned. There will be an opportunity for questions to CESPHN staff, discussion and outline of next steps on the development and implementation of these services.
