Please join us for this multidisciplinary presentation session exploring functional preservation, risk reduction and the importance of coordinated care networks for supporting older adults. We'll cover fitness assessments, fitness to drive evaluations, referral pathways and strategies for preventing frailty and keeping people safely and confidently in the community. Additionally, we'll look into range of services available, designed to support staying fit and independent in aging patients. This session also offers a valuable networking opportunity for GPs and AHPs who are most involved in the care of older adults, to share their experiences and insights as part of a multidisciplinary care providers.
