Learn how your general practice can participate in the Diabetes Care Clinic Program, a multidisciplinary initiative designed to support patients with prediabetes and type 2 diabetes.
Join this online information session to hear about the program, the expression of interest process, eligibility requirements, and the support available to participating practices, including access to diabetes-related allied health services delivered by Diabetes Australia at no cost to patients.
Eligible practices may also receive funding of up to $5,000 (excluding GST) to support participation in the program.
