Learning Outcomes:
- Understand why patients may cancel, drop-out, and fail to adhere to their treatment program by leveraging the biopsychosocial approach to patient-centred care
- Identify and re-engage potential "at-risk" patients by tracking patient outcomes and experiences
- Utilise patient-centred data to measure clinician performance and efficacy of referral sources to improve overall clinic performance
- Identify the benefits of pulmonary rehabilitation and the importance of increasing the availability of pulmonary rehabilitation for the vulnerable groups of our population
- Identify the value of referral to alternate methods of care delivery taking into consideration cost, transport and access barriers
- Identify opportunities to increase access to care with new primary care models and technologies
