This information session for Residential Aged Care Homes (RACHs) will be on the new initiative being introduced in July 2024 on MyMedicare and General Practitioners visiting Aged Care Homes. Although this new initiative is a general practice program, it is designed to support RACH and their residents in improving continuity of care and reducing avoidable hospitalisations. 

CESPHN presenters will outline how they can support GP interactions with RACH facilities and detail suggestions on how your RACH can ease the process for your residents.
