The session will introduce general practitioners to the SESLHD Primary Care Transition ADHD Service for children and young people with ADHD in South Eastern Sydney Local Health District. The session will outline the model's aims, clinical responsibilities and the supports available to GPs and practices participating in shared care or transfer of care to a continuation prescriber for ADHD management.

  4. Identify the navigating the model of care and the resources and supports available to GPs
  5. Discuss ongoing education opportunities that support the delivery of ADHD care in general practice
