CESPHN invites Residential Aged Care Homes (RACH) Clinical staff to attend an Introductory webinar on GP Connect, our new Nurse Practitioner (NP) led outreach service designed to support RACHs and GPs who visit RACHs in the CESPHN region.

The NP service is designed to provide access to GP services for residents who:

– Do not have a regular GP and need short-term GP support during admission to a residential aged care home.

– Have a regular GP who is unable to provide ongoing care (e.g., due to location or capacity).

– Have a regular GP on leave and their usual deputising service is unavailable.

The Service is designed to support clinical teams, and strengthen communication with the residents usual GP, and actively match residents without a regular GP to a permanent GP.

This session will walk you through the model, explain how referrals work, and outline how the NP integrates into your existing workflows.

Presented by: Brendan Goodger (General Manager, Primary Care Improvement – CESPHN) and Dhrupali Patel (Nurse Practitioner).

Learning Outcomes:

  1. Understand the role and scope of the GP Connect Nurse Practitioner.
  2. Identify when and how to trigger or refer to GP Connect using the ISBAR format, and in what circumstances to refer.
  3. Recognise how GP Connect can improve care timeliness, and support continuity with GPs.
  4. Understand how the NP will document, communicate, and coordinate care with both the RACH and the residents usual GP.
  5. Integrate GP Connect into daily workflows, including escalation pathways and multidisciplinary case coordination

Target audience: RACH nurses, clinical leads, NUMs, care coordinators, and any staff involved in resident clinical care, care planning or escalation pathways.
