Since 1 November 2025, POLAR has been the primary population health intelligence tool used by all practices across CESPHN. To support this transition, the Practice Progress Report has been updated to make practice data clearer and more actionable for quality improvement.

This webinar will provide a guided walkthrough of the updated report, outlining what’s changed and how to effectively use the available data sets to support improvement activities in your practice.
