Join us in person for an in-depth and interactive presentations around Southcare community service and the RADIUS clinic, a new model of care that provides rapid, responsive care to help people avoid or reduce the time they spend in hospital through thorough assessment and coordination with community services, which is the first time this model of care implemented in Australia.

Also learn about some other new and existing community facing health care services and programs available in the Sutherland Shire and St George regions to support and promote patient health and ongoing wellbeing.

Hear from service providers on care plans available, assessment options, treatment and referral pathways for GPs to improve patient outcomes and access to multidisciplinary care.
