CESPHN and the St George Hospital Integrated Care Chronic Disease Working Party are excited to bring you a monthly Grand Round series as part of the St George Hospital education program for GPs. The Integrating Care Education program is an opportunity for hospital based specialists and their respective primary care specialists to meet in a forum to discuss the care of their patients.
