The Support at Home program is a new aged-care model that streamlines in-home services by combining home care packages, CHSP supports, and assessment pathways into one coordinated system. It helps GPs by improving care coordination, strengthening clinical oversight, and ensuring older people receive timely, appropriate support – you're invited to join our lunch and learn webinar to learn what this means for your practice and patients.

• Understand what the new Support at Home Program is, what role the GP plays, and training pathways available
• Recall how services are structured under Support at Home to help inform care planning and referrals
• Identify the role the GP plays in the Restorative Care Pathway and how involving a MDT can maximise independence for our elderly population
