Case based discussion demonstrating the principles of supportive/ end of life care for non-cancer chronic disease conditions. 

We will explore how to recognise when the patient is approaching the terminal phase, where further intervention is unlikely to make a difference, Advance care planning discussions, end-of-life anticipatory prescribing, and integrating patient care between the hospital and primary care.

Learning outcomes:

• Describe the principles, indications and practical approach to anticipatory prescribing for palliative patients approaching the end of life. 
• Identify patients with advanced chronic disease (e.g. CKD, heart failure, COPD) who may benefit from a palliative care approach
• Use effective communication strategies to discuss prognosis, advance care planning, uncertainty and end-of-life issues with patients and families
