Starting 1 November 2025, the Support at Home program includes an End-of-Life Pathway to help older Australians stay at home in their final months. With up to $25,000 in flexible funding, it provides access to additional care and support for those with a life expectancy of three months or less.

This session will provide an overview of the End-of-Life Pathway, eligibility, the role of GPs and Nurses and how to access the package.
