This presentation will explore the role of palliative care in non-malignant, life-limiting illnesses such as heart failure, end-stage respiratory disease, liver failure and neurodegenerative conditions. With the use of case studies, we will explore symptom management, advanced care planning and interdisciplinary care offered by Palliative Medicine to improve quality of life of these patients.

Learning outcomes:

• Identify patients with chronic non-malignant illnesses who would be suitable for referral to supportive care services 
• Initiate symptom management for issues such as breathlessness which are common in this patient cohort 
• Initiate advanced care planning discussions with patients with chronic non-malignant illnesses who have a limited prognosis 
• Understand the role of the multidisciplinary team in managing these complex patients 
• Understand referral pathways to access palliative care support for these patients at SESLHD hospitals

The Integrating Care Education program is an opportunity for hospital based specialists and their respective primary care specialists to meet in a forum to discuss the care of their patients.
