eReferrals to Sydney Local Health District (SLHD)

eReferrals via HealthLink SmartForms are the preferred method of GP referral to the majority of SLHD services.

NSW Health’s Engage Outpatients referral management platform streamlines the triage and management of referrals, providing secure, electronic communication with GP referrers and patients on the receipt and processing of referrals.

On November 12 new eReferral forms to Canterbury and Concord Hospitals will be available as well as improvements to the referrer and patient messaging capabilities.

To accommodate the expansion of eReferral to SLHD the service listings on the HealthLink Landing page will change to:

1. Sydney LHD Hospital Services – eReferral forms for most RPA, Concord and Canterbury Hospital Services
2. Sydney LHD Aged Care, Allied Health and Community services.
